In-Network Insurances and Fees

We are happy to be in-network with Medicare. 

We are proud to be able to provide affordable, high-quality care to our community. 

As of October 2024, Vida Physical Therapy is now in-network with Blue Shield of CA. Please check your current plan for details. 

We are OON (Out-Of- Network) with Tricare Select. This means you can submit your superbill along with corresponding documentation for reimbursement. At this time, our patients have received between 80-98% of the billed amount. Please call for more details.  We are patiently awaiting full credentialing. Please check back soon for updates. 

A "superbill" is a detailed receipt provided to the patient following therapy services. It includes the necessary medical codes, description and charges to make it easier to submit an out-of-network claim to your insurance and get reimbursement. 

We would caution you to verify your out-of-network benefits with your insurance company. Some require additional forms for the patient to complete. Some insurances explicit state they will not reimburse a superbill for out-of-network services. Please take this into consideration prior to booking your session. 

Please note, we do not communicate or contact your insurance for you to verify out-of-network benefits or to expedite payment. This is a process that is the responsibility of the patient.

We cannot guarantee that your insurance will reimburse you for any services. Please ensure you become familiar with the processing.

Vida Physical Therapy makes every attempt to provide transparency to help guide in your decision making. Though we know that price is not the most important factor, it is a necessary piece of information that will undoubtedly play a role in where you choose to receive your specialized pelvic physical therapy care. Treatment fees are as follows:

Pelvic Physical Therapy Initial Evaluation Visit: $180.00

• These visits are at least 60 minutes long

Pelvic Physical Therapy Follow Up Visit: $150.00

• Follow up visits are 60 minutes

Wellness and Structured Exercise Visits: $125.00

• Visits are 55 minutes
